F-104G, construction number 683D-6110, model 683-10-19, US serial number 64-17765, built by Canadair
first flight February 23, 1965; delivery to USAF coded "FG-765"/47765 on June 25, 1965; to 3 Wing ROCAF at CCK AB coded "4338"
crashed August 11, 1971 after a tow target sortie at about 9,000 feet the avionics/electronics system malfunctioned
and the avionic bay caught fire and smoke entered in the cockpit, crashed near Jia Dong Village, Zhang Hua County, pilot ejected, Lockheed C-2 seat, written off.

During 1963 to 1965, under projects "Ali Shan No.2, No.4 and No.5" 3 Wing at CCK (Ching Chuan Kang) AB
received 56 F-104G and 13 TF-104G to equip squadrons 7, 8, and 28.
After 10 years of service, the survivors were regrouped into two squadrons, the 7th and 28th. 8th TFS disbanded in 1975.
