F-104G, construction number 683D-6080, model 683-10-19, US serial number 63-12728, built by Canadair
first flight at Canadair October 7, 1964, delivery date December 9, 1964, MAP (Military Assistance Program) to ROCAF with 28 TFS of 3 Wing serialed "4333"
crashed on November 20, 1971 at or near Hua Lian AB, following US advisor to land at CCK, pitch-up at high angle of attack, flat spin, and crashed
pilot was killed, written off.

During 1963 to 1965, under projects "Ali Shan No.2, No.4 and No.5" 3 Wing at CCK (Ching Chuan Kang) AB
received 56 F-104G and 13 TF-104G to equip squadrons 7, 8, and 28.
After 10 years of service, the survivors were regrouped into two squadrons, the 7th and 28th. 8th TFS disbanded in 1975.
