CF-104D "640" (12640) from Cold Lake, Canada in the 60s CF-104D, Lockheed model 583-04-15, Lockheed construction number 583A-5310, RCAF serial number 12640, Mk 1 (Dual Training Aircraft), built by Lockheedtaken on charge April 10,1962 at Cold Lake as Mk.1 (Dual Training Aircraft) it first went to 6 ST/R OTU at RCAF Station Cold Lake and spent its entire career there; May 1979 at Cold Lake with 417 sqn at Maple Flag; RCAF serial numbers for the CF-104 changed in June 1970 crashed December 3, 1982 with 417 OTS at Cold Lake after midair collision with 104647 on a formation proficiency mission; missing after a midair collision with CF-104 104647 of the same squadron Both pilots did not eject and were declared missing. The aircraft, both with only 1 pilot on board, crashed north of Cold Lake on January 21, 1983 the wreckage was found in and on Siebert Lake, Alberta; the aircraft was destroyed; it was a fatal crash. copyright © James Craik
